Services are offered to clients/families, lawyers, insurers and stakeholders.

 

Service Area: Services are offered in Ontario including Ottawa and surrounding areas, Kingston, Trenton, Cornwall, Pembroke, Petawawa, Deep River, Chaulk River, Carleton Place, Smith’s Falls etc.  Please call to discuss your particular location and needs.

Injury Profile: People of all ages who have sustained serious injuries such as traumatic brain injury, spinal cord injury, mental health disorder(s), chronic pain and/or physical injury may benefit from case management.

Language: Services are offered in English however, translation/interpretation services can be arranged to meet the needs of people that speak other languages.

Case Management and occupational therapy services include the following:

 

  • Discharge Planning: To assist clients’ transition from their acute medical care setting (e.g. hospital) to an alternate or community residence.  Needs are identified and medical follow-up care is coordinated within the new setting.

  •  Resources and Therapy Options: To provide clients and their support network with information regarding community resources available to them. For example, community therapists, nursing/personal care, basic home support services such as housekeeping and home maintenance, caregiver support, transportation, legal support, etc.  Funding options for these resources are also provided.

  • Timely Access to Rehabilitation To assist clients in obtaining the services they need in a timely manner. To discuss options, complete referrals and arrange funding.

  • Funding: To help clients, families and caregivers access available treatment by providing information regarding private and public funding options and assisting with the necessary application processes (e.g. paperwork).

  • Paperwork and Forms: To assist clients, families and caregivers in  understanding and completing paperwork related to insurance processes and/or community resources.

  • Rehabilitation Success and Goal Achievement: To ensure all medical and rehabilitation recommendations are followed and are in the best interests of the client, as well as ensuring that team members are working together.

  • Medical and Rehabilitation Follow-up: To coordinate required evaluations and treatments.

  • Occupational Therapy Assessments: To complete occupational therapy assessments including attendant care (Form 1), in-home safety, in-home function, work place function, caregiving, housekeeping/home-maintenance, physical demands analysis, ergonomic and return to work assessments.

  • Occupational Therapy Treatment: To assist clients in returning to pre-accident function, including return to work.
