Independent Case management Services (ICMS) accepts referrals for case management and occupational therapy services from clients/patients themselves, family members, health care providers, community agencies, insurers and lawyers.

Referrals can be sent to Liza Hadden:

• The online form can be printed and sent by fax to

• Contact Liza Hadden directly by phone, 613-324-5171 to request service or discuss service options.

All information will be kept secure and confidential.

Whenever possible, referring person(s) will be contacted by phone within 24 hours and clients will be visited within a few days of their referral or when appropriate for them.
